Also other assistance if you have been in an accident or need to apply for a passport.
Canadian Embassy in Dominican Republic
The contact details and a map to the Embassy has been added below. If you lose your passport while in the Dominican Republic you will need to report this to the police where you are. They will provide you with a notice that they have received your recording that your passport had been lost or stolen while in the country.
Take this note with the case number to the Canadian Embassy and complete the needed forms. You will need to add the case number and the police card to the application form. They will then provide you with a temporary travel document or a permanent passport if you live in the country.
Consular Assistance
There are a number of Consulates in the country. The Canadian Consulate in Puerto Plata has a map down below with their contact details. You will note that only the Embassy can provide you with a new passport as the Consulates do not have this ability. If you are getting married then the Embassy or the Consulates should be able to issue you with a letter to state that you are single.
Firstly this goes by a a number names. Its normally call a freedom to marry or a no impediment letter. You will need this letter to be certified and translated and handed in when you register your wedding. This will again be needed when you take your spouse back to Canada on a visa for your spouse.

Canadian Consulate in Puerto Plata
Firstly you will note that there is also a Embassy of Canada in Punta Cana. They can provide you with Consular assistance when needed. If you gave birth in the country you can have all the original birth documents certified and translated where there is a need. Take this to the Embassy or Consulate to register a Canadian born abroad. You will then be able to obtain a travel document for the child.
